Care Coordinator Program at FCS

You’ve got a lot going on; reaching out for help shouldn’t be frustrating and difficult.

Our Care Coordinator program is designed to help improve your overall health and well-being by providing a client-focused, team-based approach to behavioral healthcare. Our Care Coordinators are behavioral health clinicians who are ready to help you take steps to improve your mental health and your relationships and become more successful meeting your goals.

We are pleased to provide this innovative program to you at no cost. You will be partnered with a dedicated Care Coordinator who:

Works with you to complete a thorough evaluation of your presenting problem

Develops a client-centered treatment plan unique to your needs and personal goals

Coordinates your care at FCS or in our community

Helps you make sure your treatment providers are all on the same page

Provides information and answers to your questions about treatment services, insurance coverage, and options unique to you

Finds, recommends, and helps you access community resources

This service is a way to improve care for people addressing behavioral health concerns. Our care coordinator program uses a client-centered approach to improve the well-being of our clients. This means you are at the center of all we do. Your Care Coordinator focuses on removing barriers that might prevent you from being successful in your goals and making sure your treatment providers are working together to help you.

After you contact us, your Care Coordinator will be the first person you meet. The initial meeting is usually done via telehealth (on your phone or computer) but can be done in-person as well. Your Care Coordinator will help you begin the assessment process and, together, you’ll decide which FCS services are most likely to help your situation. Our intake department will help you schedule your first appointment with the Care Coordinator. Your Coordinator will then help you access other services at FCS, such as:

  1. Individual/couples/family counseling
  2. Group counseling
  3. Psychological testing
  4. Psychiatric medication evaluation and management
  5. Substance abuse evaluation and treatment

Chances are, your Care Coordinator has helped someone else with a concern similar to yours. They’re experts at helping people find solutions to all kinds of problems, not just emotional/behavioral issues. They can help with things like:

  1. Housing
  2. Insurance
  3. Transportation
  4. Childcare
  5. Employment
  6. Criminal justice involvement
  7. School success
  8. and more.
You will have regular contact with your Care Coordinator. You can expect to have easy access to your Care Coordinator via telehealth, phone, or email. They’ll meet with you in-person when you need to as well. You can expect your Care Coordinator to help you solve any problems you may have with your progress in treatment at FCS. You and your Care Coordinator will work together to decide on how often you’ll meet.
As long as you’re receiving services at FCS, you’ll have access to your Care Coordinator.
If that’s a service you need, you’ll get it at FCS. Most of our clients see their therapist more frequently than they do their Care Coordinator.
You’ll be expected to meet with your Care Coordinator as often as the two of you agree. You should let your Coordinator know if you’re having problems with any of the services you’re receiving. Contact your Care Coordinator if something big changes in your life and you need help in a new area.
